External Speaking & Third-Party Engagement Disclosure and Approval Form

This form is required for all SBDC at FGCU employees, contractors, and consultants seeking approval to participate in externally hosted events, speaking engagements, workshops, panels, or similar activities that occur outside direct SBDC programming and may intersect with employee duties or create real or perceived conflicts of interest.

Florida SBDC services are extended to the public on a nondiscriminatory basis. Language assistance services are available for limited English proficient individuals.
